Symposium 10: Sustainable Healthcare Practices (Cardiopulmonary)

Beyond Discharge: Redesigning Cardiopulmonary Physiotherapy from ICU to Community

Learning objectives:

By the end of this symposium, participants will be able to:

1. Identify key rehabilitation and care gaps experienced by cardiopulmonary patients transitioning from ICU to community care.

2. Apply patient-centred education and health literacy principles to improve safety, engagement, and long-term self-management in chronic cardiac and respiratory conditions.

3. Integrate sustainable, cross-setting strategies into cardiopulmonary physiotherapy practice across the continuum of care.


Symposium Convenor: Ms Mohena Priyaa, Senior Physiotherapist, Woodlands Hospital

Biography: Priyaa is a senior physiotherapist and cardiopulmonary team lead practising in an acute hospital setting in Singapore. She has experience in critical care, complex medical and surgical patients, and cardiopulmonary rehabilitation across ICU and general ward environments. In her leadership role, she is actively involved in service development, clinical education, and the mentorship of physiotherapists, with a keenfocus on safe, evidence-based practice. Priyaa has a particular interest in respiratory muscle training, early mobilisation, ventilator weaning, and interdisciplinary collaboration to optimise patient outcomes. She regularly works alongside intensivists, surgeons, nurses and allied health professionals to translate evidence into practical clinical workflows. She is committed to advancing clinical excellence through education, collaboration, and the continual integration of evidence into practice.


Speaker: Mr Emmanuel Farrock, Senior Physiotherapist, National Healthcare Group (Jurong Health Campus),Ng Teng Fong General Hospital

Title: Acute / ICU Perspective (title coming soon)

Biography: Emmanuel is a Senior Physiotherapist and Cardiopulmonary Team Lead at Ng Teng Fong General Hospital, Singapore. He provides rehabilitation across ICU and general ward settings, working with patients from medical, surgical, and neurological populations. In his leadership role, Emmanuel is actively involved in service development, enhancement of clinical education resources, and oversight of daily cardiopulmonary team operations. His clinical interests include lung ultrasound, early mobilisation, and physiotherapy outreach services to upskill frontline clinicians and improve post-ICU patient outcomes. He works closely with intensivists, nurses, respiratory therapists, and allied health professionals to strengthen interdisciplinary care and advance physiotherapy practice within the healthcare system.

Speaker: Ms Anna Teo Mei Lun, Principal Physiotherapist, St Luke’s Eldercare Pte Ltd

Title: Community Perspective / Airmaster Programme (title coming soon)

Biography: Anna is a Principal Physiotherapist specialising in cardio‐pulmonary care. She has extensive experience across the continuum of care, from intensive care units and outpatient services in acute hospitals to her current role in the community. This breadth of practice has shaped her special interest in bridging cardio‐pulmonary care between hospital and community settings. She has collaborated with hospitals to establish referral pathways that strengthen transitions after discharge, ensuring patients receive seamless and holistic rehabilitation. At this conference, she will share insights on advancing cardio‐pulmonary rehabilitation through collaborative models, highlighting strategies to improve continuity of care and patient outcomes.

Speaker:  Dr Judy King

Title: Patient Education Perspective (title coming soon)

Biography: Dr King is a physiotherapist and associate professor in the physiotherapy program at the University of Ottawa in Canada where she teaches the cardiorespiratory component of the program.

Her innovative person-centred research program encompasses both patient education and education of health care students and professionals. Her fields of study include chronic disease management, health literacy, interprofessional education and clinical reasoning. She has been privileged to present her research work at many international conferences.

Dr King is grateful to be the recipient of a number of awards for her teaching and mentoring of students and new health professionals including a Chair in University Teaching.

She has been appointed to several national and international committees including being a founding executive committee member of the International Confederation of Cardiorespiratory Physical Therapists (ICCrPT) of World Physiotherapy and currently is an executive member of the World Physiotherapy’s Education Committee.
